What is Home and Community Care Services?
The City of Casey, Community Care Department provides a range of comprehensive Home and Community Care (HACC) based services for frail aged, people with a disability, and their carers. These services aim to work with and support client’s independence and self-efficacy for them to live at home and to participate in everyday activities. These services aim to build capacity and is not an emergency service.
Am I eligible?
If you wish to access Home and Community Care Services you must undertake an assessment by a Community Care Assessment Officer.
To be eligible for Home and community Care services you must meet the following:
- Requires a basic level of care
- Living within the City of Casey
- Meet the eligibility criteria within our services guidelines
- Older persons with moderate disability/illness, needing assistance
- Younger persons with moderate disability,
- Individuals who are at risk of premature or inappropriate admission to log term residential care
The outcome of an assessment is subject to assessed and prioritised need.
How do I access HACC services?
You, your doctor, a family member or friend can contact the Community Care department to make a referral to our services
After a referral has been received an Assessment Officer will visit you at home to assess your needs and to develop an individual care package in consultation with the client, their family and/or carer, and arrange for the most appropriate services.
The following services are available:
Fees / Payment
All services have a small fee which is charged to a monthly account. Your income will be taken into account when determining individual service charges.
Volunteer transport has no set fees, but we do ask for a contribution towards the cost of transport. Home Maintenance services will incur a charge for the cost of materials used.
