What are Home and Community Care Services?

The City of Casey provides a range of comprehensive Home and Community Care (HACC) services for frail aged, people with a disability, and their carers. These services aim to work with and support client’s independence and assist them to live at home and to participate in everyday activities.

Am I eligible and how do I access HACC services?

You, your doctor or health professional, a family member or friend can contact the City of Casey’s Community Care Department to make a referral to our services.

Short and long term services are provided, depending on your goals and the assistance you need to maintain an active daily life. Services are also available to support people who care for an older person or a person with a disability. A small fee usually applies.

If you need assistance, a Casey Assessment Officer will discuss your situation with you to determine if you are eligible for these services and may arrange a home visit to discuss your specific needs.

The information and questions we ask you relate to your health, medical, cultural, social, language and other care needs. This helps us decide with you how our services can best assist. If you prefer to communicate in a language other than English, you are able to have an interpreter present at no cost. If you require an interpreter, please advise the Assessment Officer in advance.

The outcome of an assessment is subject to assessed and prioritised need. If you need more support than we can provide, information can be provided to you about other services, or, with your permission, a referral made to another agency on your behalf.

Community Buses

Community Transport Service can help disadvantaged groups and individuals in Casey that require transport

Home Care

To maintain independence while living at home, carers can assist with essential house cleaning tasks

Home Maintenance

Helping eligible frail aged clients and clients with disabilities to maintain a safe home

Meals on Wheels

Nutritious three course meals for eligible clients are available for home delivery by a Casey volunteer

Personal Care

Assistance with bathing, dressing, meal preparation and more

Respite Care

Activities for people being cared for

Social Support

Social Support Programs are for older people and people with disabilities
